"Your Baby Is Measuring Big" Cool, Cool, Cool. Let's Talk About How Accurate That Actually Is.
I say this to clients constantly, so I'm just going to say it here too, in writing, so I can link to it forever:
Ultrasounds are a tool. They are not a crystal ball.
I have had clients told their baby was measuring way too small who delivered a totally average 7.5 lb baby. I have had clients told their baby was measuring way too big who also delivered a totally average 7.5 lb baby. Same outcome. Wildly different amount of stress, worry, and "well maybe we should induce/schedule a c-section just in case" conversations along the way.
So let's actually look at the data, because I don't want you to just take my word for it.
The Question I Actually Went and Answered
Are ultrasounds accurate at telling you whether you're going to have a big baby?
I went digging through the research so you don't have to. Here's what's actually out there.
What the Research Says
First, the general accuracy of a weight estimate at all: A systematic review looking at 36-week ultrasound scans found they were accurate — meaning within 10% of the actual birth weight — about two-thirds of the time. Flip that around: roughly 1 in 3 scans is off by more than 10%. On an average baby, that's nearly a full pound of wiggle room. On a "big baby" prediction, that's the difference between a totally normal delivery and a scheduled induction.
Now, specifically for "your baby is measuring big" predictions: A large population-based study published in the American Journal of Obstetrics & Gynecology found that of babies who were predicted to be large for gestational age on ultrasound, 58% were not actually large once they were born.
Read that again. More than half.
That's not a rounding error. That's basically a coin flip with slightly worse odds than a coin flip.
And it's not just the "big baby" predictions that miss. The same study found that when it came to babies who were actually small for gestational age at birth, 49% of them had been called normal-sized on their ultrasound. So the errors go both directions — babies get flagged as big when they're not, and babies who are genuinely small get missed entirely.
And when researchers looked specifically at whether "big baby" ultrasounds could predict actual delivery complications — like shoulder dystocia, which is often the real concern behind the "big baby" conversation — a massive meta-analysis of over 112,000 patients found that an estimated fetal weight over 4,000 grams only caught 22% of shoulder dystocia cases. It missed the vast majority of the actual complication it's supposedly screening for.
So... Why Does This Matter to You?
Because "your baby is measuring big" is not neutral information. It changes conversations. It can nudge you toward an induction you didn't necessarily need. It can make you doubt your body's ability to birth your own baby before you've even tried. It can turn the last few weeks of your pregnancy — which should feel like anticipation — into low-grade dread.
And "your baby is measuring small" can do the opposite kind of damage: unnecessary extra monitoring, extra appointments, extra anxiety, extra intervention conversations — for a baby who, statistically, has close to a coin-flip's chance of showing up perfectly average.
I'm not saying ignore your ultrasounds. I'm not saying providers are doing anything wrong by giving you the estimate — it genuinely is useful information, and there are real situations (diabetes, certain risk factors) where it matters more and performs better. I'm saying: hold the number loosely.
What I Actually Tell My Clients
An estimated fetal weight is an estimate. It has a real, well-documented margin of error — not a vibes-based one, an actual, measured-in-research one.
"Big" and "small" predictions are wrong more often than most people realize — over half the time, in some of the largest studies we have.
If a "big baby" or "small baby" conversation is driving a decision about induction, delivery timing, or mode of delivery, it is completely reasonable to ask your provider: how accurate has this specific estimate historically been, and what's the actual complication risk we're trying to avoid here — not just the number on the screen.
Your body, your baby, and your birth are not a math problem with one right answer plotted on a percentile chart.
Ultrasounds are one piece of information. They're a genuinely helpful piece, most of the time. But they are not the whole story, and they are definitely not a guarantee — no matter how confidently that number gets said out loud in the exam room.
