What Actually Triggers Labor? Here's the Honest Answer.
I get asked this one a lot: "What actually starts labor?"
And the honest answer? We don't fully know. Not in the tidy, satisfying way we know what causes, say, a fever.
What we do know is that it's not just your body deciding one day that it's time. There's growing evidence that your baby plays an active role — that as their lungs and brain mature, they send out hormonal signals (things like cortisol and surfactant proteins) that travel through the placenta and start a cascade back to you. That cascade shifts your hormone balance, softens your cervix, and eventually builds into contractions.
So in a way, your baby is telling your body, "I'm ready." But the exact mechanism — how that signal fully works, and why it happens when it does — is still an active area of research. Even researchers who study this for a living will tell you the full picture is still, in their words, a bit of a scientific enigma.

And the honest answer? We don't fully know. Not in the tidy, satisfying way we know what causes, say, a fever. There's no single switch that flips, no one hormone we can point to and say "there — that's the moment." And I think that surprises people, because so much else about pregnancy gets explained to us in neat, confident terms. This is one of the places where the truth is a little messier, and honestly, a little more amazing because of it.
It's Not Just You
For a long time, the assumption was that labor was purely a maternal event — your body, on its own timeline, deciding it was done. But the research over the last couple of decades tells a different, more collaborative story.
There's growing evidence that your baby plays an active role in starting their own birth. As their lungs and brain mature in those final weeks, they begin producing hormonal signals — things like cortisol and specific lung proteins (surfactant) — that travel through the placenta and start a cascade back to you.¹ That cascade shifts your hormone balance, softens your cervix, and eventually builds into contractions.
So in a way, your baby is telling your body, "I'm ready." It's not a passive process on their end at all — it's closer to a conversation happening between the two of you, hormone by hormone, in the days and sometimes weeks leading up to labor.
The Pieces We Do Understand
While the full picture is still incomplete, researchers have mapped out several pieces of the cascade pretty well:
Fetal cortisol rises as your baby's adrenal glands mature, and this appears to be one of the earliest signals in the sequence.
The progesterone-to-estrogen shift — progesterone helps keep the uterus calm and relaxed throughout pregnancy, and as that hormone's influence "steps back" near term, estrogen's influence grows, making the uterus more responsive to contractions.
Prostaglandins get released locally in the uterus and cervix, helping the cervix soften and ripen, and working alongside oxytocin to build contraction strength and frequency.
Oxytocin receptors in the uterus increase dramatically in the final stretch of pregnancy, so the same amount of oxytocin has a much bigger effect than it would have earlier on.
Individually, each of these is fairly well understood. What's still unclear is exactly how they're all coordinated — what starts the sequence, what determines timing, and why it happens when it does for one pregnancy versus another.
Why This Is Still "Unclear"
I love sharing this one because I think we do pregnant people a disservice when we act like every part of the process is fully mapped out. It's not.² Even researchers who study this for a living will describe the complete picture as still something of a scientific enigma. There are competing and complementary theories — some focused on the fetal signals I mentioned above, others looking at inflammatory changes in the fetal membranes, others at a "placental clock" that may help program the length of pregnancy itself. None of these are mutually exclusive, and most researchers think the real answer involves several of these systems working together.
Your body and your baby are doing something remarkably complex together, and science is still catching up to all of it.³ That doesn't mean it's random or scary — it means it's genuinely one of the more fascinating unanswered questions in human biology.
Why I Think This Matters for You
Here's why I bring this up with families: when we don't understand a process fully, it's tempting to try to control it — to look for the one food, the one exercise, the one date on the calendar that will "make" labor start. And I get it. The waiting at the end of pregnancy can feel endless.
But understanding that labor is a complex, multi-system, baby-led process (not a single switch you can flip) can actually be really freeing. It's not something you're doing wrong if it hasn't started yet. It's not something you can rush by sheer force of will. Your baby's body is doing real, measurable, biological work behind the scenes, on their own timeline, and your body is responding to that in kind.
So if you're in the tail end of pregnancy right now, waiting: your baby isn't stalling. They're maturing. And when they're ready, they'll tell you — even if science hasn't fully cracked how yet.
